Illustration of a conveyor drive end with an open gearbox/belt-and-pulley drive on a green frame at left, a yellow mesh guard panel mid-span, an E-stop pull-cord with red button along the top, and a brown fluid pool on the floor beneath the drive.

Clear, evenly lit stylised illustration of the conveyor drive end; small details such as switch bodies and fixing threads are simplified, limiting judgement on interlock hardware.

Leak, drip or product stainingVC-LEAKyesconfidence 92%

Floor directly below the blue drive unit at left, with spatter trailing right. A brown/amber pool with scattered droplets sits under the drive, with a dark line running down from the underside of the blue gearbox/motor into the pool.

Accepting this evaluatesR-GM-17

Guard removed or rotating part exposedVC-MISSING-GUARDyesconfidence 88%

Drive enclosure at left end of conveyor. Drive pulleys, belt and gear wheel are fully visible in the open within a black-backed frame with bolt holes/fixings at each corner and along the edges, no cover panel fitted.

Accepting this evaluatesR-GM-01

Housekeeping around the assetVC-HOUSEKEEPINGyesconfidence 90%

Floor beneath and to the right of the drive end. Spilled liquid pool roughly 1 m across with scattered droplet spatter spreading across the floor slab.

Interlock defeatedVC-DEFEATED-INTERLOCKunclearconfidence 50%

Open drive enclosure edges and mesh guard mounting. No interlock switch, key or actuator is depicted at either guard position, so no judgement on defeat is possible.

Other observations
  • Yellow mesh guard mid-span appears mounted on a single central pedestal and stands clear of the belt line, leaving the conveyor underside open.
  • Red pull-cord runs the length of the conveyor to a yellow E-stop station above the belt, appearing intact.
  • Mesh guard is positioned on the operator side only; no guarding visible at the far end rollers.

Recorded on the report as notes. No cue, so no rule consumes them.

4 cues checked and not seen
  • Heavy corrosion or section lossPainted steelwork and legs appear uniform in colour with no flaking, scale or thinning visible anywhere in the frame.
  • Light surface corrosion or coating breakdownNo staining, rust bloom or chipped paint depicted on the painted surfaces; drive pulleys are plain grey.
  • Working space obstructedFloor is clear of pallets, drums or stock; only the fluid pool occupies the standing area.
  • Impact damage or deformationAll members read straight and parallel; mesh panel is flat with no dents or bent sections visible.
